The "Rules"

Eligibility: the 50/50 Pay Day Lottery (the "Lottery") is only open to participants who are active employees of Hotel Dieu Shaver Health and Rehabilitation Centre and a deduction has been taken for the pay period being drawn.
How to enter: Employees must be enrolled in the Lottery using the form above and submitting the form to the Foundation Office in order to participate and be eligible for the draw. $5 (per ticket) will be deducted from each pay period of the employee as entry into the Lottery, until the employee notifies the Foundation in writing. The minimum sign-up for the Lottery is 12 months. Employees will not receive a paper ticket.
Only payroll deduction is a valid payment for purchasing lottery tickets. Lottery tickets may not be purchased with personal cheques, credit or debit card, or other form of payment. Lottery tickets may not be sold or given to any other party.
All Lottery forms must be given directly to the Foundation Office. All forms must be submitted by the 15th of the month to be eligible for participation commencing the following month.
All purchases of Lottery tickets are FINAL and NON-REFUNDABLE. No portion of the ticket price may be deducted as a charitable contribution for federal income tax purposes.
Prize/Payment: The Lottery prize consists of and is limited to a cash prize equal to fifty percent (50%) of the net Lottery ticket sales revenue. A cheque in the amount of the prize will be provided to the prize winner within fifteen (15) days after the Foundation has verified the prize winner has satisfied all terms and conditions of these Official Rules, including all forms and documents. No substitutions, refund or transfer of a prize is permitted.
Drawing: One winning Lottery ticket will be randomly drawn by the Foundation the following Friday after payday. The Foundation reserves the right to publish and/or notify the winner by phone or in person, by announcement over the HDS public announcement system, and posting on E-Span and the HDS/Foundation website(s).
In order to claim the prize, the winner must go in person with proper identification to the Foundation Office during the hours of 8:30am - 4:30pm, Monday through Friday, within three (3) weeks of being declared the winner. The winner must collect their winnings in person at the Foundation Office. if the winner does not claim their prize within the allotted time, the prize money will be donated to the Foundation.
General Conditions: Participation in the Lottery constitutes the winner's consent for the Foundation to use the winner's name and photo for promotional purposes in any manner or media without further payment, notice, or consent. The Foundation reserves the right, it its sole discretion, to terminate, modify or suspend the Lottery at any time. The Foundation is solely responsible for the operation and conduct of the Lottery.
